(SCAMP-HDM) senior Community Action Meals Program-Home Delivered Meals provide home-delivered meals and outreach to isolated seniors allowing them to maintain their independence. Participants receive a hot meal daily, weekend meals, visits by a registered dietician, informational material on good nutrition, and are monitored on a regular basis to determine changing needs.
